How it DOES work: Some NPC are faking death, where they should not.

How it SHOULD work: they are quest NPC, so they shouldn do that

Examples are Sentinel Shaya in Teldrassil and one of the BattleMasters in Darnassus.

From Shaya I know that she was sitting on the ground in previos versions.

It looks more like a DB bug, i think you should contact PSMDB team.

After updating Mangos to 10526 und SD2 to 1828 the bug is gone
