mangos two Death Knight Quest Broken,... HELP!

There is a broken quest right at the beginning for death knights. Quest 12848 where you unchain an Unworthy Initiate NPC & kill him does not unchain the NPC or change it's faction so I'm able to kill it. Anyway I can get some help fixing this?

It is already scripted in SD3, however there is an obvious issue with it...
